*To register you on the platform we need your consent. This will be presumed when you fill in the survey link the required data. Filling in the survey is necessary to arrange your access to the epi learning platform. We collect the data of the survey and upload it in the epi learning platform. After this is done, an email with your login credentials will be sent to you.
